Social care is any care, support or interventions needed to carry out day-to-day tasks which you may be finding difficult. This could include help with managing money or improving relationships, transport to attend appointments or assistance with benefits.
We will work with you to provide effective interventions to help improve social issues that may be affecting your mental health and wellbeing. These are essential for maintaining both mental and physical health and can come in the form of understanding advice, guidance or practical assistance.
We may also put you in touch with alternative services who will be best placed to address your needs as fully as possible. We offer a range of group-based and one-to-one interventions.
We may offer an initial assessment to discuss your social needs. This helps us to identify the best ways to support you, address your needs and to achieve the best possible outcomes.
We offer a range of different social support such as access to psychoeducation and peer support groups, where you will be provided with knowledge, information, and resources to improve your mental health. Groups are also a great way to connect with others and your local community.
We will connect you with local experts who can help with the many external factors that contribute to maintaining mental and physical health such as money, work, social and leisure activities and benefits.
